Pierce County is Hiring a Case Manager (Medicaid) Near Tacoma, WA

This position is eligible for 10% field work specialty pay for hours worked. 
Special Note:This is a grant-funded position. Continued employment is contingent upon job performance/evaluation, continued funding of the program, and other appropriate County procedures.
The first 3 months in office, then scheduled levels of telework until probation is complete.

Pierce County Human Services is the community and human services arm of the Pierce County government. It is the largest contributor to Pierce County's safety net providing services and funding to nearly 170 agencies and governmental jurisdictions. The department collaborates with its community partners, including other public and non-profit funders and service providers to understand current and emerging community and human services needs. How to be successful in the role: 
Develop proficiency with administering CARE Assessment tool, authorizing services, and tracking financial eligibility for our clients.Core Daily Responsibilities:
  • Authorization, oversight and monitoring of older and disabled client care plans, along with prompt response to changes in client's condition and care needs.
  • Provide comprehensive assessment to determine any individualized client needs using a laptop in the client's home setting
  • Respond to individuals seeking information by providing routine information, referrals and assistance.
  • Monitor both the progress of the client and the adequacy of the services being provided.
  • Provide coordination of services and advocacy for older and disabled adult clients.
Core Strategic Responsibilities
  • Completion of annual CARE Assessment, 6-month home visits, quarterly client monitoring contacts.
  • Prompt response to client-related phone calls and emails, generally within 2 business days, which aligns with the Human Services Strategic Plan.
  • Frequent communication with clients, family members and community providers to ensure services are being provided in accordance with the established care plan.

  • Bachelor's Degree in related field.
  • 2 years case management experience.
  • Or Master's Degree in related field with 1 year case management experience
  • Possession of or ability to obtain a valid Washington State driver's license within 30 days of employment or otherwise meet the travel requirements. 
  • Successful completion of a thorough background investigation, in accordance with RCW 43.43 is required prior to employment.
